Missouri · Tax account closure

Missouri Form 5633, explained.

Form 5633 is Missouri's Final Report, the Department of Revenue voucher that closes a sales and use tax account, an employer withholding account, or a single sales tax location when the business stops, sells, or stops paying wages. The voucher itself says a final return must still be filed, even with no tax to report.

Missouri DOR, Form 5633

State fee. $0. The Final Report carries no filing fee.

What the state asks for

  • Which tax types are closing: sales and use tax, employer withholding, or both
  • The Missouri tax identification number, business name, and owner name
  • The date the account or location closed
  • The reason for closing: out of business, sold, no employees, or other
  • The physical address when only one sales and use tax location is closing

How long it takes

Missouri publishes no processing time on the voucher. The account is closed when the Department stops expecting returns for it, which is why the final return matters as much as the form.

What we do

We prepare the Final Report, confirm the final returns that must accompany it, file both, and watch for the account to stop expecting periods. The unemployment account is closed separately in UInteract so the notices stop from both agencies. You keep the client. We never email them.

What is Missouri Form 5633?

The Final Report: the Department of Revenue voucher that closes a Missouri sales and use tax account, an employer withholding account, or one sales tax location, filed when the business is out of business, sold, or no longer has employees. It mails to the Taxation Division in Jefferson City.

Does Form 5633 replace the final return?

No, and the voucher says so in its own words: you must file a final return, even if you have no taxes to report. The Final Report tells the Department the account is closing; the final return closes out the last period. Skip the return and the account keeps generating non-filing notices.

Does Form 5633 close Missouri unemployment tax?

No. The voucher's checkboxes cover sales and use tax and employer withholding only. The unemployment account belongs to the Division of Employment Security and closes in UInteract, which is why a complete Missouri payroll shutdown is two closures, not one.

Missouri DES, UInteract

Can Form 5633 close just one location?

Yes. The voucher has a section for closing a single sales and use tax location, asking for that location's physical address, so a business keeping its other Missouri locations open files the same form.

What does Form 5633 cost?

Nothing. The voucher has no fee line.
